I agree to pay my account in full upon receipt of the monthly statement. Any account not paid by the end of the month will be considered over due and a 2% finance charge (24% annual percentage rate) will be added to said account. By signing below, I ask that an account be opened for me. I understand that you may verify and exchange information on me, including requesting reports from credit reporting agencies. I am aware that this information is used to determine my eligibility for credit, and if my application is approved you may contact the sources to update this information at any time.
In the event of default I/We will pay all collection costs and any attorney’s fee of one-third of the amount due. I/We will notify you immediately of any change in ownership or operations.
I/We personally guarantee payment in full including all finance charges, collection costs and attorney’s fees incurred as specified above, and waive any presentment, demand, protest, and any other notice regarding this guarantee of payment.
